Illawarra In Home Support provides Community Aged Care Packages to frail aged people living in the Kiama, Shellharbour and Wollongong Local Government Areas. A Community Aged Care Package is equivalent to low-level hostel care provided in a person's own home. The types of services that may be provided as part of a package of care can include:
It is important to know that the services provided can change, as care needs change.
Community Aged Care Packages are for frail, aged persons who have been assessed by an Aged Care Assessment Team (ACAT) as needing low level hostel care. We provide the equivalent of hostel care in their home. We cannot accept self-referrals.
Client charges are according to Commonwealth Department of Health and Aged Care guidelines. Fee is 17.5% of single aged pension. Contributions for service are negotiated with the client when services are organised and no one is refused service.
The Illawarra Aged Care Assessment Team based at Port Kembla holds the centralised Illawarra Community Aged Care Packages waiting list. To book an assessment please contact them on 1300 792 755.
